I have a customer who is experiencing some annoying issues with thier 7941 phones. When a call comes in, it is automatically answered and goes straight through to the speaker with no user interaction. It is not a user profile issue as the phone can be replaced and work ok. plus different users can login with a similar result. Has anyone seen a similar issue? Is it a firmware issue or a hardware?

Auto Answer is configured in the IP Phone itself?

What is the CCM version and phone firmware load?

Check the Directory Number settings of the Line in question on the physical phone. Auto answer can be -
1 - Off
2 - AutoAnswer with Handset
3 - AutoAnswer with Speakerphone
