1131 AP and WLC Disconnection Issue

I'm having an issue where the 1131 AP seems like it is connecting to the 4402 controller, but about 2 minutes later, it is disconnected. Here is the last few lines of my debug output. This happens to all APs. Controller is running 3.2.150.10. 1131 is running 12.3(11)

What could cause this to happen?

The controller and AP must maintain a heart beat[Just Like Keepalives] every 30 seconds.After this 30 secs after 5 attempts,one second each, that the AP will be de-registered.So check for the link between Access Point and WLC.

Did you get a resolution to th eproblem because I am seeing the same on our AP 1200 that is in a remote site to teh WISM.

pls how can i console into air-lap1131AG wireless access point for configurations? and what is the default password to it?
