I have an NM-NAM installed in a 3845. Does anyone know if the external interface can be configured for 100Mb / fixed / full-duplex?

This is currently not possible, nor is it on the NM-NAM roadmap. However, it will be a good feature to have, so I will raise a feature request for it.
