Media List exhausted normally means that allocation of a a Media resource (In this case MOH) failed.
THis can be normally for 2 reasons, No MOH sessions available for the time stream was requested or no MOH servers were available for that request.
I would open a TAC case to check in detail.

If RTMT shows that you have resources available, then try solution mentioned at http://forums.cisco.com/eforum/servlet/NetProf?page=netprof&forum=Unified%20Communications%20and%20Video&topic=IP%20Telephony&CommCmd=MB%3Fcmd%3Dpass_through%26location%3Doutline%40%5E1%40%40.1dde11bb/0#selected_message
If that does not work, then you might want to open a TAC case
