I am trying to configure a SIP to SIP gateway so I can get a SIP trunk through the firewall. Do I use Dial Peers to direct the traffic? The document gives me basics but not specifics.

Yes you use dial-peers voip for both inbound and outbound. Generally speaking, you will have "codec transparent" set on these.
You will need and IP-to-IP image to do that, filename ends in _isv, but you probably knew that already.

No, I didn't know this. I am using a 2821 router running this image: c2800nm-advipservicesk9-mz.124-11.T1.bin. Will this work?

No I don't think it will work. Anyway, another needed piece of configuration is:
voice service voip
allow-connections sip to sip
Then, to see how things are going, "term mon" and "debug ccsip message" when placing a call.
