I have been getting the following pop-up upon opening a new window.

"Authentication with TACACS+ ACS failed. You are in the fallback mode.

To revert to CiscoWorks Local mode, you have to run:

NMSROOT/bin/perl NMSROOT/bin/ResetLoginModule.pl.

You must have appropriate privileges on the system to run this script.

Note: NMSROOT is the product installation directory."

How do I go about configuring my TACACS+ ACS server as well as CW to avoid this?

Here is a write up that was original produced by TAC that takes you step-by-step through basic LMS/ACS integration:

This should take care of this problem for you.
