My current setup is:
2851 Gateway with a 4 fxo card installed
PBX phone line plugged into port 0/0/0
CCM v 5.1
I can call any internal office number and any off site local or long distance number from my IP Phone through the PBX with no problems but when I try to call an IP Phone say DN 3000 I get one ring then a busy signal.
I tried adding the PLAR 3000 command to voice-port 0/0/0 and that did not help.
I have all my route patterns running on the CCM
I am using MGCP

Could you please do a CCM trace and see whether the dialled didgits are sent with the appropriate prefix

Well the thing is that because from the point of view of the PBX the router is just an extension, PBX will never outpulse any digit.
And I think the connection plar won't work because you have configured MGCP.
If you want to make a serious bidirectional integration with the PBX, you need to use some digital card like ISDN BRI, PRI, at very minimum E&M.
Or, configure CCM to that when PBX rings the FXO port, it does something. The alternative is H.323 for the gateway, and have use connection plar (opx) to dial a single extension.
