I'm noticing that when new user are setup in unity, sometime the email address does not created in exchange. when this happen user can't get receive voicemails. could someone explain to me why this happening and what I can do to prevent it.

This could be several things to look at, mostly Exchange / AD related.
1) Check to make sure you have Exchange SP2 or any of the latest service packs / patches for your Exchange server
2) Try adding a test account, go to the Global Catalog server and force an AD replication
3) Could be that the accounts take some time to get replicated if there is a Domain or Enterprise policy causing problems
4) While adding the test account, monitor Event Viewer and see if it spits out any errors.
5) As a last resort, you could try rerunning Permission Wizard again
