Are the messages that you posted slightly truncated? Is it really User failed to log in from C?

Is something connected on the console port and is it possible that something is resting on the enter key? The messages indicate that it believes that someone is attempting to login, gets 3 attempts which are not successful, and reaches max attempts, then it starts over. Something on the enter key or a stuck enter key would produce these symptoms.

to add to Rick's good advice, a router or other device connected to the console port, with the functionality of "inverse access server", that has echo and/or EXEC active on the line, would also produce the same result.

Hope this helps, please rate post if it does!
