CUPS1.0.3, error on 'upload own cert'

I'm working the CUPS 1.0.3 deployment guide and get a problem on page 21, step 8.

in step6-h, I left the default certificate template to 'Administrator' and got the following error when I upload,

"The file cannot be uploaded because of the error: CSR SubjectName: CN=DEMO-CUPS.ucdemo.com does not match Certificate SubjectName: EMAILADDRESS=#161841646d696e6973747261746f7240756364656d6f2e636f6d,CN=Administrator,CN=Users,DC=ucdemo,DC=com

Please fix the error and try it again."

If I change the certificate template to 'Web Server', the subject type is okay but still error as follows with blank reason,

"The file cannot be uploaded because of the error:

Please fix the error and try it again."

How to resolve? advise please,

resolved by,

-certificate template=web server(page20, step6-g)

-certificate name=CA's name. not sipproxy.pem(page21, setp8-d)
