Caller to Unity or IVR hears MoH

I have an odd situation where a caller to either one of our IP-IVRs or to Unity hears a music on hold stream after connecting. I can hear the IVR/Unity prompt in the background, but it is choppy and unintelligible.

Any ideas as to what might cause this phenomenon? I'm thinking there may be some issue with Media Resources somewhere. Could a MTP that shouldn't be enabled cause this?

I have this problem too.
