CME 4.0 - 7961 + 7914 Is Not Working

I am trying to setup a 7914 module on a Cisco 7961 phone using CME 4.0(3)... here is all the relevant configuration:

I've tried a million things and I feel stumped. The 7914 is lit up but all of the keys are RED and nothing is showing on the display. Yes I've checked power and the serial link on the back and everything is connected properly.

Appreciate the help.

Hi, all seems ok, pls enble term mon and defug tftp events, reset and check output.

Never mind, it works this time.

I reset the phone, holding down the # key, it got new FW and now it works.
