Wasted 4 days on this last month.

If you have ASDM 5.2 & IPS 6.x then you can directly reset the password from the ASDM ( Tools, IPS password reset )

Can you tell us what is the version of the IPS ?

If it is greater than 6, then the hw-module password reset will work, if it is 5.X you have to reimage the AIP SSM as above link
