We have a location with a 4 port FXO card with 4 POTS lines. They started having problem where they can't make calls. We tested the 4 POTS line and they all work.
Any ideas on what is causing this problem

So you have the proper dial peers configured?

I had FXO card lockups every month or so on two 2811's that were testing CME 3.3 back in late 2005. When we moved them back to the version the router shipped with, CME 3.2, the lockups went away.
By the way, the only thing I could do to reset the FXO card was to reboot the router. None of the shut/no shut commands did anything.

I ended up rebooting the router and upgrading 12.4(11)T3
