Is this a new install ? the server or any services been reset ? what exact CallManager build?

Just to follow up, it is new install, server and services were reset. CM 5.1.2.3000.

TAC fixed the issue, it was a corrupt iptplatform directory. You have to get root access to CM and delete the iptplatform directory. Then stop Cisco Tomcat service and restart the service. The iptplatform directory gets rebuilt after restarting the service.
