I have found a bug related to this IOS with the same problem. The bug ID:CSCsg00183. According to the bug the resolution is to upgrade to 12.4(13.11)T.

Nice find. I didn't catch that it was happening when I did a 'show call history voice' until I turned on terminal monitor.
