what is the function of the optional keyword extendable in: ip nat inside command ?

A translation entry which maps one IP address and port pair to another.

but the translation works even if the extendable word does not exist

The keyword means that there may be other translations between the same pair of addresses, using different ports. Without the keyword, only that entry would work, and no others between those addresses.
If I remember rightly, the keyword is inserted automatically if you specify the TCP and/or UDP port on the translation. Which means it isn't really optional - it is just information.
