Unknown Unicast

I have question regarding a unknown unicast and how exactly they occur. Would a unknown unicast occur by the mac-address being aged out on the switch but the sender still having the correct arp entry?

Thankyou in advance for any insight you may have.

There are a couple of ways that unknown unicast (or unicast flooding) can be caused, which are detailed in this link (and yes, MAC addresses being aged out can be part of the problem):
