Since there is no upgrade path from IPCC 4.0(5) to 5.0(1) does anyone know of a way to export the personal phonebooks for the configured agents and then reimport them into the new 5.0 install? I have no problems recreating everything, but my users will not be happy with recreating their phonebooks again.

Try using the Global Phone Book in CAD.

Do you know where the phone books are stored on the server because I am having a hell of a time finding them.

To answer your question I remember reading Cisco FAQs and I believe they said there is no way to export the personal phone books from the agents.
