that means that the client exclusion feature is enabled on your WLAN (enabled by default, per WLAN) and the particular user has failed 802.1X authentication 3 times. The exclusion policies are defined under "Security > Wireless Protection Policies > Client Exclusion Policies"

why would they typically fail 802.1x authentication 3 times usually ? just bad passwords ? i seem to get this a loads
