VG248 MWI issue

I have a remote site with a VG248 with the following setup:

Software Version: 1.3(1)ES8.2

MWI Method: Lamp

On-Hook Voltage: 13V

Expected Voltage: 90V

We do not understand why we are only getting 13V for On-hook. The On-hook should be 90V. The result is that we never see the MWI LAMP LED turn ON for the analog phone connnected. Is this a known issue?
