I have a couple dozen 7961 and 7941 phones that are "stuck" in upgrading firmware and continuously download new firmware from the TFTP server after a CCM upgrade to sr6a (CCM 4.1(3)). I've tried resetting the phones to factory defaults with no luck. Anyone have suggestions how I correct this?

recreate the TFTP files, or try hard-coding the device load on the device page

I've tried hard coding the phone load @ the device level. It did not help. I beleive this is a phone problem not a CCM problem because if I change the MAC address on the device page, the new device will boot fine.
