Problem with c3750 switches

Dear all,

One of our friend is facing following issues. They are using 3750 with 12 fiber ports and have had too much problems. Here's an extract

'During their maintenance window they have upgraded 3 Catalyst3750G-12S switch stacks . The 2 most crucial ones were upgraded without problems. After the upgrade of the third stack, its second stack member failed to come up due to a corrupt flash.

They replaced 3 Catalyst 3750 switches :

SW-1 reason: corrupt flash after upgrade

SW-2 reason: preventive replacement due to switch stack hung while this one was master (happend twice in a 2 week interval)

The incidents with 3750G-12S over the last 2 months show 3

switches that ended up brain dead either after a power down/up or after a upgrade/reload.

One year back a total of 6 out of 10 switches were replaced:

As all their distribution switches are C3750G-12S-E stacks(currently 6 , 2 more to be added shortly)

Problems with bugs in the past: If one stackmember failed to boot, 3 out of 12 ports became active, and the entire stack disappeared in OSPF in the core switches. connectivity lost to half of the terminal…

If any one you have faced similar issues kindly discuss so that we can resolve the matter.

It might be due to bad Power supply and this is simply a hardware failure. Flash failure few times and this is alarming

and this also happening at times due to the wrong procedure of upgrading the switch/Stack.

Do you have a reference link for upgrading switch/stack in a proper fashion !!
