busy voicemail

I have a UC 500 system in place. The customer mentions that when they forward to VM, they sometimes get a busy signal. Whether the person is on the line or not doesn't seem to matter. Any ideas on why this would be happening?

You can do "show ccn call application all" from the CUE module to see active calls.
