how do you upload CER database to PSAP?

After completing CER database, how do you get it to the PSAP?

I have a U.S. legacy PBX that has E911 and it had to have the following: (1) a E911 monthly service added to existing PRIs and (2) a modem and software to upload changes. Do I have to keep either/both of these for CER to work?

CER allows you to export the ALI database into NENA standard format, however what you will find out is that most carriers want this in their own propriatary form. You need to contact your service provider and ask for e911 service, this is by all means not free, you will be charged to upload the ALI database. I suggest going with the lite product as with CER you will most likely not need subscription based service unless you will be adding more zones.
