OPS Manager not provides Switch port details where the IP Phones are connec

We are running LMS 2.0.1.

IP Phones are connected to cisco WS-C4510R-E switches. We can able to generate reports of IP Phones,but without the port details of switches where the IP Phones are connected.

And in Dash Board>service level view>Switches with IP Phones connected , it not displays any switches.

Pls help me out..
