Outgoing voice low volume And Sometimes cannot transfer calls

I have a 2851 with PRI and some 7921 phones.

1. Have an issue with low volume of voice going out, incoming is fine.I have tune the "output attenuation",but it make any different.

2.Sometimes(maybe 10% opportunity),call cannot be transfered,when on call,the "Transfer" key is not functioned

Any idea about these?
