for 1 user IB calls go straight to v/m. OB is fine.
Any help is appreciated.

check the transfer settings for that users subscriber to have it set to ring the subscriber's phone

We can check that with traces enabled, or by u telling us how is the call.
Meaning: Call comes from the pstn, hitting and mgcp, h323 gateway, going to CCM (version) then phone "A" should ring and instead goes to VM.
or when hitting a callhandler from unity and dial by extension goes directly to VM.
That's how you tell the call flow, so as JAVALENC said, What's the call flow?

1. From my desk I call his extension. call goes to vm.
2. calls from PSTN hit h323 GW, then ccm 4.1.3, should ring at x6543, but directly to vm

you might have replication issues, try setting and unsetting CFA on that DN

Can you please restart the DBL layer monitor service on all your servers and see if that helps.
Also checked if the phone does not have the callforward all to VM.
