I have 5 unity servers (version 4.something). A subscriber was removed from server #5, which removed it from the global subscriber tables on all of the other servers except for #4. dbwalker doesn't see it, so is it safe to just kill off that row by hand or are there references in another table somewhere that I also need to remove?

If this subscriber does not exist in the Subscriber or GlobalSubscriber table on any other server, it is safe to manually delete this subscriber.
Afterwards, you can use ADSI Edit to ensure the following attributes have been cleared in Active Directory for this user:
ciscoEcsbuDtmfld
ciscoEcsbuListInUMDirectory
ciscoEcsbuObjectType
ciscoEcsbuTransferId
ciscoEcsbuUMLocationObjectId
