Off the wall question, but does the Call Manager support extension numbers that begin with 0 or 9

Sure, but you need to design your numbering plan wisely.
For example, if you have extensions that beging with 9 and you also add Route Patterns that beging with 9 (like 9.@) then you will have a delay (interdigit timeout) while placing PSTN calls.

CUCM takes any digits from 0 thru 9 for extensions, no limitations on that. Just as Jorge mentions choose wisely and take your time to define the dial plan
