WLC Web login page

A couple people i work with came up with a nice Wireless Disclaimer page that goes over a few rules with a "agree/disagree" button. My question is, does anyone know how to make this functional? I have uploaded it to the WLC, and set it where once you open a web browser, it defaults to this page to "login". How can i make those buttons functional? Lets say you hit agree, it will let you right in but if you disagree, it will deny access to the web.

You need to make sure you keep the hidden html code there. Then the buttons will work.

Could you elaborate a little more? They designed the page, but the buttons don't work. I need to find a way to make the buttons work.

Here is the Cisco default WebAuth page. Take a look at the script and the hidden fields. You need to keep most of this code, except for the picture and background color.
