Hi
I am using CCX 5.01 and when agent finishes call and starts workflow, she gets the following error
The agent or workflow initiated action request failed.
Any thoughts?

i'm seeing the same error message. CUCM 6.01 and UCCCX 5.0(2) build 64

Hi Jesse,
Could you please explain the issue in more detail. Like what is the call flow (what Agent is trying to perform here), also details of the Workflow defined in CDA, also post the workflow along with the Agent configurations.
Would also request you to please post the Agent logs to see what is really going on. Agent logs fan be found at
C:\Program Files \Cisco\Desktop\log
