CallManager Phone Template not valid

We are trying to use the Bulk Administration featuer to add phones. When we try to add phones, we get the following error message:

00222555f8dd9 jeyoung7911 is not a valid phonetemplate

We have looked all over and cannot find an answer as to what CallManager is looking for. We have created a phone template named "jeyoung7911". Is the problem with the template itself?

Is this jeyoung7911 a BAT Template? It almost sounds like a Phone Button Template.
