CVP transfer using CTI Route Point access fails

We use CTI route points which to transfer calls to outside partner numbers. This is done for reporting. The routing ICM 7.1/IPCC script uses CVP 4.X to transfer the call. Agents use the route point to transfer callers to these partner numbers. The transfer fails giving dead air sometimes. A reset fixes the issue. Sometimes a dis-associate and re-associate with the ccm jtapi pguser fixes the issue. How do we fix this sporadic issue? What do you recommend to trace these calls?

If it works, then fails at times, I would look into making sure you have the latest Cisco patches for all components.
