HTTP Policy doesn't appear to work for all connections

Can someone explain to me why I might see a connection (in this case port 80) being optimized for some connections but not others? My http policy states that destination port 80 get's optimized, and I see it for some connections and not others. Running 4.1.1a

What is the status of the connections that are not being optimized? Are they is pass-through?

If I look at the connections stats it says there are no policies applied.
