It looks like corrupted flash memory. Try to redo the image upgrade. This time, choose to erase and format the flash memory prior copying image to the flash.
If you simply delete an old version of IOS and upload a new version, the verify command will not automatically check the hash. This means that to get an automatic verification, you must erase the flash, upload IOS, verify IOS, and then upload all the SDM files.
Also it seems to be a filesystem issue, when the flash is formated into low end file system (#erase flash:); once the flash gets formatted in DOS file system (#format flash:) we are able to verifying the flash.
