I've looked at 'Understanding Debug isdn q931 Disconnect Codes', but need further evidence to point to Meridian PBX issue.

Log files and debugs where error is seen (debug isdn q931 / show log)
show tech
Description of call flow when error is seen
Cause i = 0x81A9 - Temporary failure, is coming from your PBX or TELCO (whoever is controlling the first upstream ISDN switch).

Sometimes with PRI circuits we have to guess why the other side is disconnecting. The best way to troubleshoot this is to simplify the trunk as much as possible, and then start changing the more 'core' features.
If you're looking to get this fixed with the greatest efficiency start with the Meridian, because temporary failure is a code that is equivalent to a '500 Internal Server Error'. This message is sent on IOS gateways when we have DSP errors for example, or when we specific bugs we hit.
In CCM you can try changing your QSIG encoding type in the service parameters, and uncheck any extra check boxes on the MGCP gateway page.
Otherwise, you will want to ask Meridian what's going on with their circuit.
