This one is easy. Use overlay lines in CME before 4.3/7.0, or Octolines after that.

12.4(20)T or later looks like this:

ephone-dn 1 octo

number 555

ephone 1

mac

button 1:1

ephone 2

mac

button 1:1

CME guides on Octo lines:

Overlay DNs for IOS before 12.4(20)T or 12.4(15)XZ:
