We are running Unity 4.2.1 and Exchange 2003. We've recently had a few users who will receive deleted voice mails in the inbox. Typically, the voice mails span a few months and just randomly appear in the inbox again. Thus the user thinks they haven't been receiving any voicemails. I'm thinking this is more an exchange issue but wanted to know if anyone else has seen this.

Chris,
You may want to double check that Message Store Manager tool isn't moving Deleted Items back to the Inbox on a certain schedule.

We don't use the MSM. Any other thoughts?

are there any emails returning to the inbox?

Like Randy suggested, is this happening to emails as well, or just limited to voice mails? Maybe that will help determine if there is some sort of task on Exchange moving a particular header type around...

Take a look at:

Looks like Blackberry syncs may be responsible for doing it?

If you look in your Commserver/UnityMTA directory, are there maybe old messages that got stuck there long ago that are getting delivered after the fact?

No messages were there (I have alerts to notify me if there are any). Most or some of the messages were already listened to and deleted but showed up again in their vm account.
