On this Unity 5.0(1) server I can open Internet Explorer and login to any of my 4.1(3)SR6 CallManager subscribers/publisher viahttps://server/ccmadmin or https://server/ccmuser and login with PhoneViewAdmin (AXL Manager) or PhoneViewUser (CTI Manager). I've reset passwords several times.
I've set this up in the past with no issues. Here are the log files;

If you haven't tried already, change the Authentication URL on CUCM/CCM to use the IP address rather than the name. Also, be sure that another device isn't configured there as we've seen paging systems configured which if not configured properly to handle Unity's requests will cause it to fail.
How many digits are the extensions in which you're trying to implement phone view with? May want to also take a look at:
