DHCP error with 6509

I am using 6509 core switch in which i configure dhcp pool for every vlan from this i connect 4510R using as a distribution swtch from this i connect 2960 switches as an access layer. My client machine is conneted with 2960 the issue is that some times my Pc of vlans get the Ip address and after 3 or4 days they do not get and showing error limited connectivity but i have sifficient ips for each vlan. Kindly provide the solution its critical. i am attaching the log of every switches(Core,Distribution,Access layer)

Did you enable spanning tree portfast on the access port of 2960 switch? If not, enable it to see if it makes any difference.
