Being placed on hold

We have remote users that when they place a call from a remote phone (home office), it immediately places them on hold. This does not happen all the time, randomly to various users. Any ideas? The only common thing we found is MOC, but not why. TIA

This is because the CallManager receives a CtiLineCallHoldReq from the MOC. Here the OCS is requesting the call to be put on hold because the status from the previous call did not clear the "In A Call" status from CUPS in OCS.

Well, if the phone is sitting idle, and it is used, could be the first call of the day, as soon as the called party answers, the calling party phone goes to hold? It could have been an hour since the last call.
