Is it possible in CUCM 5.1 to add a name to an external number based on their Caller ID?

This is info you get from the TELCO. Not sure what type of GW you have, but you can check this link, you'll need MGCP for that.

Do you have PRI? H.323 or MGCP ?

We have a pri and running H323.

With Cisco CallManager, the caller name is not displayed on IP phones in an inbound call from an H.323 gateway even if facility IE is available. Since Facility IE and Display IE are provided by the Telco we won't be able
to modify the changes for this. You can get in touch with your Telco and
inquire if they can provide you the Display IE instead of Facility IE.
Alternatively you can use MGCP gateways as well.

Please take "debug isdn q931" with "term mon" to see if and how the calling name is delivered by telco.

Have you tried looking at this product to see if ot is what you are looking for. Not supported by Cisco but can be effective
