There are two options:
1) Set up SIP on your gateway. Then, set up either H323 or SIP to CUCM. This is CUBE.
2) Set up the SIP trunk directly on your CCM. Since you're on CCM 4.2, this isn't recommended since SIP support on that version isn't very good.
MGCP is a protocol that allows CCM to control analog endpoints. In the case of a SIP provider, you need either SIP or H323 to send calls to the gateway since there are no analog endpoint involved in the topology.

Can your provide a sample on gw piece?
Is it better better to use sip or h323 to CUCM?
Sounds like with 4.2, it is better to use h323 btwn CUCM and GW and from SIP btwn GW and provider.

I'm running CUCM 4.2(3)sr4b and have implemented a SIP trunk to a 2811 CUBE router to connect our system to another non Cisco platform. It works flawlessly. Just make sure you build your dial peers and dial plan correctly and you won't have any issues.
