High Windows Output Queue Length

We're having an issue where we are seeing high output queue lengths on many of our windows server hosts connected to a 3560 switch. The queue will often times be above 5 or 10 deep with .01% utilization on the NIC.

The servers have been swapped, drivers updated, cables and ports changed, but the problem still exists. Is there anything on the switch that may cause this? Our next step is to upgrade the ios, but it's on our production DMZ, so I want to rule everything out before we drop the network.

I note many hard code speed/duplex ports. There are no duplex mismatches between switch and servers?

PS:

Could you also indicate an actual problem port and post switch stats for that port?
