We have an issue where a caller calls in over the PSTN.
The call is answered o.k and both parties hear eachother. They then transfer the caller to a user on a different phone system ( A siemens PBX in this case). The ip phone user can consult with the transferee but after they have mad ethe transfer to the caller on the PSTN. neither party can hear eachother. (PSTN caller - Siemens phone) the problem is, it works occasionally, they seem to think it only fails when there are a lot of calls.
I am leaning towards an MTP issue on the Gateway, any ideas?

Whats the topology?
Which type of GW u use from pstn and which one to PBX.
Make sure IP routing is correct between PSTN GW and PBX GW.

A Cucm 6 connected to 2821 gateway to pstn.
a trunk to a callmanager 4.2 which then connects to a 2651 gateway which is a Qsig connection to PBX.
All routing is fine
