WS-X6348-RJ-45 not responding on Cisco 6506

I installed a WS-X6348-RJ-45 into a Cisco 6506 switch on slot 3. My IOS version is s72033-pk9s-mz.122-18.SXD5.bin.

The card keep giving the following errors below.

hello Olaosebikan,

try to reseat the linecard

try it in another slot

if the behaviour doesn't change ask for replacement

"Major online diagnostic failure" sounds like a hardware failure.
