I installed a WS-X6348-RJ-45 into a Cisco 6506 switch on slot 3. My IOS version is s72033-pk9s-mz.122-18.SXD5.bin.
The card keep giving the following errors below.

try to reseat the linecard
try it in another slot
if the behaviour doesn't change ask for replacement
