UCCX Call Dropped

Hi, There has been an instance on the UCCX 5.0, where a call was dropped mid conversation. I have review the MIVR logs and attached is the output. I can see the agent 5104208, extension 5995906 went not available. Any ideas why this is, I would like to stop it from happening again. Thanks

What version of CUCM is CRS integrated with? Are you aware of any gateway issues with the CM? How many NIC enabled on this server?
