Getting the following error:

What can I do to resolve this issue?

Sounds to me like the config / IOS is corrupted. Have you tried the TFTP recover procedure yet? If you know that the AP was previously running IOS and not something older like VXWorks, then this should take care of it...

1. Connect to the following link..

2. Read the section where it says "Reloading the Access Point Image."

3. You'll need to download and rename one of the IOS files in order to do this. I just use the 12.3(8)JEA image file, but technically you can use another IOS....

That's not an IOS image problem, it's the radio firmware. Different issue.

Try reseating the radio module. If that doesn't help, replace it. That's a pretty old piece of hardware there; it's no big surprise it's starting to act flaky.

I recommended doing that because if you've ever upgraded a VXWorks AP to IOS, it does indeed flash the firmware on the B radio. If the card was not detected by the AP it would say a radio interface is not present; not necessarily that the firmware is mising. I've never seen that before. I'd say it's worth a shot, since it will rule out the possibility of the AP missing files....
